PRIMIS GRASP-COPD & Asthma Care audit tools


GRASP-COPD and Asthma Care audit tools are both free from PRIMIS and are easy-to-use tools that assist GP practices to interrogate their clinical data. The tools enable practices to improve patient outcomes, reduce costs and avoid inappropriate treatment for patients with COPD or asthma.

Both audit tools work with CHART (PRIMIS’ software tool, which is also free) which many practices in the East Midlands already use. After the audit tools are installed and run, results can be displayed in a number of formats, such as a practice-level dashboard summary or a datasheet which has the facility for you to drill down to an individual patient level.

No special templates or different ways of recording data are required and training to make best use of the results can be arranged with a PRIMIS expert. How can this help me? This audit tool can help practices by:

  • including a case finder to identify patients who may have a missing diagnosis code for COPD and identifying those at risk of developing the disease
  • enabling GP practices to easily audit their management of patients with COPD against NICE Clinical Guideline 101 - Management of chronic obstructive pulmonary disease in adults in primary and secondary care
  • enabling practices to check a patient's severity against how they are currently being treated, helping them to ensure that patients receive optimum care as well as highlighting potential savings (by reviewing treatments that are not clinically indicated)
  • helping practices manage their patients with COPD and giving a graphical representation and 'snapshot' of patient care
  • linking results to NHS Outcomes Framework Domain 1 (preventing people from dying prematurely) and Domain 2 (enhancing the quality of life with people with long term conditions)

How can this help me? This audit tool can help practices by:

  • generating a list of patients with possible asthma and providing relevant information to help clinicians to confirm or exclude diagnosis helping to establish a more accurate prevalence rate for asthma within their practice population
  • facilitating clinical audit against national standards for all asthmatic patients prescribed asthma medication within the last twelve months
  • summarising practice achievement of the Royal College of Physicians ‘3 questions’ outcome measure which assesses asthma patient wellbeing
  • using patients’ medication history to summarise treatment strategies based upon the British Thoracic Society/Scottish Intercollegiate Guidelines Network (BTS/SIGN) stepwise approach
  • highlighting patients whose current treatment step may require review
  • reporting on key factors that are associated with an increased risk of exacerbation
  • contributing to the delivery of the Quality and Outcomes Framework, the NICE quality standards for asthma (QS25), the NHS Outcomes Framework (NHS OF), the Clinical Commissioning Group Outcomes Indicator Set and the Public Health Outcomes Framework (PHOF)
